Hypothyroidism questions

Hello doccc,
A year and a half ago I had a check for TSH because i was feeling very tired and i was sleeping too long(I know this symptoms can be from other things too).I came up with TSH a little bit elaveted from the upper limit.My doc said lets recheck it after some months(he thought that it might be from stress).When i did a recheck TSH was like 8-μΙU/ml.Then,i took 50mcg of T4 (Levothyroxine) daily and after some months I rechecked my TSH which it was within the normal range (but not 1.0).Also that blood test had every other marker that someone could do for thyroid and everything was normal even the autoantibodies.My doctor said you will take for 6 months again 50 mcg and then you will stop it for 1 month and recheck.When i stopped taking T4 after 3 weeks I started sleeping again a lot.Yesterday, I did the blood work and today the results was, elevated TSH 6.905 mIU/ml (upper limit 4.940) , FT4 0.99 ng/dL (Limits : 0.70-1.48), PTH 23.1 pg/ml (Limits:15.0-80.0).My questions are:

1.Can I have hypothyroid without elevated autoantibodies?
2.How are we determing the dosage?I mean I took 50mcg of T4 and then i was between the normal range but for example If I had taken 75mcg i could also be within the normal range.Are we choosing the dosage based on how I feel?If that is so,I can feel tired from other things too.

I have an appointment with my endocrinologist next monday.

  1. Yes, you can have hypothyroidism without high autoantibody titers.

  2. It’s usually a combination of labs (specifically TSH/FT4) and symptoms. If you have normal labs and persistent symptoms, you should look for other reasons to explain fatigue (including, for example, a sleep study for obstructive sleep apnea).

Endocrinologist said to take 50mcg of T4 and recheck everything at the end of summer to correct the dose.
