Is the knee rehab template appropriate for a 41 yr old with 20 yrs chronic knee pain?

I’ve had knee pain in my right knee for 20 years. I don’t recall any injury except one really hard fall on it when I was a teenager. I don’t think it was that because it didn’t start hurting until years later. The pain came on gradually. I’d say it took about 10 years to reach a climax. It hasn’t gotten any worse in the last 10 years.

It’s not bad enough to keep me from being active. However, after I do anything active it generally hurts afterward. It’s often especially bad after intense exercise like barbell squats or HIIT training on an exercise bike. Although even that seems somewhat random. Sometimes I’ll have squat sessions where it feels better afterward.

Yesterday I squatted and deadlifted. Today I did some steady state cardio on an exercise bike. Right now if I put pressure on it, like sit to standing, it feels like a hot knife is stabbing the area right behind the knee cap. Maybe a little to the right (outside).

About 10 years ago, I had it looked at by a naturopath. He used a portable ultrasound and determined that there was no loss of cartilage at the time. I’d say the pain now is about the same as it was 10 years ago, and I’ve done lots of barbell squats since then.

More recently, I had it looked at by an ortho, a couple of years ago. He didn’t x-ray it. He moved it a few ways, pressed on it a few ways, and suggested I only do partial squats with light weight. He sent me to PT for a few sessions, and nothing they did made any difference.

Some days it doesn’t hurt at all. I’m usually very perplexed on those days, because I can never pinpoint anything I did to make it feel better. It’s especially perplexing if it hurt badly the day before.

All that to say that I don’t think it’s from an injury, but…?

About me: 41, 6’3, 310lbs (35%bf)

My training history: For about 5 years, I’ve lifted weights off and on. I’ve bounced back and forth between SSNLP and a beginner bodybuilding program I like. Most recently I’ve been running SSNLP again. I think I’m nearing the end of the novice progression as things are getting pretty hard.

Would the knee rehab template be a good one for me to run right now?

However, the template does not address HIIT or dynamic loading.
