Preventing Minor Back Pain/Injury

Generally all of my pain and discomfort over the last few years of lifting has been some sort of back pain. Does anyone have recommendations on preventing or minimizing various back injuries? I often am dealing with a rhomboid “tweak” or an erector spinae “tweak.”

Typically I feel the rhomboid pain when benching (squeezing scapula together to keep a solid upper back). And when I tweak the erector spinae (fairly high on the back) it’s something like low bar squat and trying to keep a really tight straight back.

I know muscular injuries are fairly common in powerlifting, but are there any solid preventative measures to minimize minor things like this?
