Shoulder injury, forward posture, winged scapular, one side

Hey guys so I’ve been reading here a while and have some questions about my shoulder injury. Im 28 years old and weigh 155lbs. I’ve been in amazing shape the last ten years. Rock climbing at a pretty high level (5.13), jiu-jitsu for about 5 years and competing a handful of times. On top of that I do construction everyday so needless to say I was in great shape. I could walk over a hundred metres on my hands and was working one arm push up’s and pull up’s.

This time last year I was grappling with a friend after a few beers and the next day I had some pretty bad pain when lifting my arm away from my body. Didn’t notice it that night, but like I said, beers. So for the next couple weeks I was really babying it at work, not even using it much. Then I slowly started using it again more and more It eventually got back to almost 100% after around 6 months. Still a bit painful in weird angles and positions though but it didn’t bother me 99% of the time. I will say I noticed it looked a little bit different than my left shoulder from that injury though. Just barely but noticeable to me in the mirror.

Fast forward to 2 months ago and I had a small fall off a longboard and caught myself with my right arm. It didn’t hurt like the first time and I thought I was fine so I went home and worked the next day. It started around this time maybe a week later but I started experiencing pain when lifting my arm away and up from my body again.

So I’m thinking I’ve damaged something for sure I just don’t know what and am looking for opinions or someone who’s been through something similar. I’ve seen two phyio therapists who don’t seem to think anything is wrong, just certain muscles/tendons are weak or damaged (one said subscapularis, other said biceps tendon and serratus anterior) and they give me some exercises to do (resistance band stuff) but I’m not seeing much progress it still hurts a month after seeing them. I’ve seen my doctor in the meantime (I’m in Canada) and have a MKSultrasound booked but it’s 50! Days away from now and I’m going a little nuts not knowing what I can do and what the damage could be or if it’s not a big deal and I should be working it out etc.

So where I’m at now is my right shoulder (injured one) feels like it’s sitting closer to my body than the left and rolled more forward. This causes pain so I use my back muscles to pull my shoulder back but that hurts after a while too. I’m going to make a quick video showing the difference in the two shoulders and explaining a bit more. I’m not looking for a diagnosis, just opinions and maybe experiences and maybe some suggestions. I still have to wait for my scans and who knows how long it will be. I’m trying to take care of it in the meantime. I’m going to call another pt today to get a third opinion because I don’t nessarily trust the two PT’s I’ve seen they don’t seem to know how to take care of their own bodies let alone mine.

I’ve googled probably too many things like most people and have feared some pretty bad outcomes already. I’m hoping nerve damage isn’t the case. Anyway I’ll post a link to video below showing what I’m talking about. Thanks everyone and have a great Sunday.

The pain I experience is mostly under the bottom part of my right shoulder blade. But if I work out with my arm too much I start feeling other pains in different parts of the shoulder. When sitting it feels like my right shoulder blade is poking out so it hurts to sit in chairs to long. One last question I have is if I end up needing some type of surgery do you think it’s been too long? Because initially I hurt it about a year ago and now this bad pain has been here for more than a month, maybe two. And I may not know what’s wrong for another couple months. I just hope I didn’t wait to long and healthcare in Canada takes forever as it is.
