SLAP Labral Tear w/ partial-thickness tear at the biceps labral attachment

Hello Rebab Team,

I really appreciate your opinion(s) as to whether you would recommend surgery or conservative “good” rehab for this injury. Here are the MRI results on the right shoulder post arthrogram on 1/12/2023.

Impression:

  1. SLAP labral tear with intra-articular biceps tendinopathy and partial-thickness tear at the biceps labral attachment. There is no full-thickness tear, subluxation or avulsion

appreciated at this time though the functional integrity is in question.
2. Insertional supraspinatus and infraspinatus tendinitis with subinsertional enthesopath

change, but no high-grade partial thickness or full-thickness rotator cuff tear.
3. Mild hypertrophic/inflammatory AC joint arthropathy.

I have taken the time to read previous forum posts regarding SLAP tears (as well as the 4 part shoulder articles), but none of the previous posts also had a biceps tear. The MRI report does mention that the intra-articular component is diffusely frayed and heterogeneous in signal with a high grade partial thickness tear at the labral attachment.

Therefore, I really appreciate your thoughts! I have been in contact regarding a rehab consult, but I wanted to get some initial thoughts first.
