WHI follow-up study

Do you have a view on the new WHI study The Women’s Health Initiative Randomized Trials and Clinical Practice including the advisability of HRT and calcium/vitamin D supplementation, and under what circumstances they might be advisable, generally or stratified by age?

My view is that this study corroborates existing evidence and guidelines pertaining to hormone replacement therapy in women, e.g. that it probably doesn’t reduce risk of chronic conditions like heart disease, dementia, diabetes, osteoporosis, and so on. In general, HRT can be used to manage symptoms of menopause in low-risk individuals within ~10 years of menopause onset.

Vitamin D and calcium supplementation has not been demonstrated to reduce risk of most conditions either in this population. The recommendation to supplement if dietary intake is insufficient is likely reasonable, but whether or not that does anything in otherwise healthy populations is unclear. In general, vitamin D and calcium supplementation is not recommended in this population.
