Managing Osteochondritis Dissecans of the Elbow

Hi there, I’ve had osteochondritis dissecans in the elbow for 25 years (since age 13, am now 38). I was finally diagnosed 2 years ago through MRI imaging. They found what looks to be a 6mm loose body as well as an osteochondral lesion, some arthritis. I’ve always had crepitus in the joint, but it hasn’t gotten worse. I can usually fully, or almost fully, extend my elbow to 0-2 degrees. Typically it can not fully go into normal flexion (so for example, I can’t touch my right shoulder with my right hand, but can do so easily with my left hand). It hurts if I force it.

I started weight training four years ago, and it has gone relatively well for the elbow. No worsening of symptoms. Long story short, I fell on my arm two weeks ago. It fully compressed under my body’s weight, the way that it doesn’t want to. The elbow joint immediately flared up. It definitely feels like there is something loose in there now. Sometimes it “catches” and I feel a sharp pain when going into flexion. I’ve gone to the gym a few times, and I’m feeling a bit lost now as I don’t know if heavy lifting is appropriate. Everything I want to do revolves around the elbow functioning properly. Even squats will require me to hold the bar behind my head. I’m going to consult a doc to see if I need surgery, but in the meantime, I’d like to continue strength training.

How should we manage strength training situation where there is a mechanical issue? If the joint is inflamed, should I still be moving it as much as it will allow? I tried a curl and tricep kickback session today, and it was sore, pinching, badly. I tried a machine variety and it seemed “ok” if I limited my range of motion. Crepitus was worse than usual while bench pressing. I’ve lost range of motion in extension by about 10 degrees as it’s so tight from the inflammation now. Should I try to deadlift with an arm that doesn’t want to fully extend due to inflammation? Should I “wait it out” for a week with just plain rest?
