Sleep Disorders and Low Libido/Erectile Dysfunction

Hello Doctors;
I am a 26 y.o. male (6’2", 200lbs, +5 years resistance training) who has more recently been experiencing erectile dysfunction. In addition this, I have experienced low libido, difficulty sleeping, daytime fatigue/inability to focus. After discussing this with my physician, we scheduled a blood test (CMP, CBC, lipid panel, testosterone), and he prescribed a trial of sildenafil (viagra). In short, there were no red flags from the blood test, and I have not experienced significant differences when using the sildenafil. I rarely get spontaneous erections anymore throughout the day, which leads me to believe that the problem is not purely psychological/anxiety related.
I have heard that there is a relationship between ED and sleep disorders, so I was wondering if it would be worthwhile to look into a sleep study. As far as sleep apnea goes, I sleep on my side and have never had anyone report me snoring/choking in my sleep. According to the STOP-BANG questionnaire, I might be at risk due to the following factors:
-Tired/Sleepy: (subjective, but something I have experienced, especially now that I am catastrophizing about ED).
-Blood Pressure: Said yes, although mine is only slightly elevated, (when measured with a cuff at home, my systolic is usually between 120-135mmHg. Diastolic is well within normal).
-Being male

Triglycerides 74
HDL 70
LDL 84

Testosterone:
total testosterone: 974
free test: 90.8
bioavailable testosterone: 186.7
SHGB: 57 (slightly high)

TSH (thyroid stimulating hormone): 0.87 (normal range 0.36-3.74)
Hemoglobin A1c: 5.2 (normal below 5.7)

Test was taken around 10:00am while fasted

Thanks for the post and I hope you’re doing otherwise well. Just a note, posts like these with very specific medical questions, medical tests, etc. are firmly within the scope of professional consultation and I would advise others in the future to consider a consultation with us or their physician. We will be unable to answer most of these types of questions specifically due to lack of information and medicolegal concerns.That said, a few thoughts:

  • I would agree that a referral to a psychiatrist is warranted in these situations
  • The IIEF may be useful for detecting clinically relevant changes
  • blood pressure is elevated if it’s > 120, which would likely respond well to lifestyle changes
  • Sleep studies should be driven by clinical evaluation and relevant instruments. the STOP BANG questionnaire can be useful in addition to other tools regarding parasomnias.

I think the first two items here would be highest on my priority list.
