Is BFR an effective training strategy for tendinopathy?

In combination with activity modification and grade exposure, couldn’t BFR potentially yield greater results while training at lighter loads?

My short answer to this, without typing out a dissertation, is “no.” The magnitude of loading seems to be the integral component to what would be deemed positive adaptation with tendinopathy.

BFR has been gaining traction as a panacea for training through an injury but the evidence has not caught up to support its utility. Much of the case for it is based from extrapolating physiological studies which has not worked out well in the long run in the past. A quick pubmed search only showed one trial:

The problem with this in general tendon physiological adaptation is slow so even at 14 weeks I would not put much stock in the changes noted.
